This form is used for parishioners of Holy Spirit who have been asked to be a sponsor for a baptism or confirmation. This is NOT an auto-fill form. Please download, print, complete the form, and either return the form in person to our parish office or email it to Becky Messimer, Secretary, at

BULLETIN REQUESTS: Requests to publish information in our bulletin should be submitted electronically to Becky Messimer, Parish Secretary and Bulletin Editor, two or three weeks prior to publication. Your bulletin request may be edited for clarity or space constraints. Email Becky at with the following information:

Name and date of event
Date(s) of bulletin publication
Contact name and phone number and/or email address
Ministry/organization name
Description of event
