BAPTISM OR CONFIRMATION SPONSORSHIP FORM: This form is used for parishioners of Holy Spirit who have been asked to be a sponsor for a baptism or confirmation. This is NOT an auto-fill form. Please download, print, complete the form, and either return the form in person to our parish office or email it to Lori Jasper, Secretary, at firstname.lastname@example.org.
WEDDING FORM: Please complete this form if you wish to marry. Once we receive your information, we will contact you.
BULLETIN REQUESTS: Requests to publish information in our bulletin should be submitted electronically to Lori Jasper, Parish Secretary and Bulletin Editor, two or three weeks prior to publication. Your bulletin request may be edited for clarity or space constraints. Email Lori at email@example.com with the following information:

Date(s) of bulletin publication
Contact name and phone number and/or email address
