PARISH REGISTRATION FORM: Download and print out the form, complete, and submit either at the Parish Office or drop in the collection basket during Mass.
BAPTISM OR CONFIRMATION SPONSORSHIP FORM: This form is used for parishioners of Holy Spirit who have been asked to be a sponsor for a baptism or confirmation. This is NOT an auto-fill form. Please download, print, complete the form, and either return the form in person to our parish office or email it to Lori Jasper, Secretary, at firstname.lastname@example.org.
BULLETIN REQUESTS: Requests to publish information in our bulletin must be submitted to Lori Jasper, Secretary and Bulletin Editor, two weeks prior to publication, using this form: Bulletin Publishing Request. This is not an auto-fill form. Please print, complete the information, and return to the parish office.
Or, you may email Lori at email@example.com with the following information: Name and date of event; date(s) of bulletin publication; contact name and phone number and/or email address; organization; and description of event. Your bulletin submission may be edited for clarity or space constraints.
